Pathways Healthcare in Westwood, MA, is seeking a Registered Nurse - Hospice to provide patient-centered home health and hospice care in Taunton, Attleboro, and surrounding areas. You will develop and revise care plans, administer medications per physician orders, and coordinate with families and interdisciplinary teams.

Ideal candidates have 2+ years of nursing experience (geriatrics preferred), a nursing degree, and solid EMR skills. A valid driver's license and reliable vehicle are required.

Qualifications

• 2+ years of nursing experience, geriatrics preferred.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Valid driver's license and own reliable, insured vehicle.

Responsabilités

• Assesses and evaluates patient status; writes and initiates plan of care.
• Regularly re-evaluates patient and family/caregiver needs.
• Participates in revising the plan of care as necessary.
• Develops a care plan with goals based on nursing diagnosis and palliative actions.
• Initiates preventive and rehabilitative nursing procedures; administers meds per orders.
• Completes and submits accurate clinical notes; tracks pain/symptom changes.
• Communicates with physician regarding needs and reports changes.
• Maintains a safe environment for the patient.
• Attends interdisciplinary group meetings.
