UnitedHealth Group is seeking a Hospice Registered Nurse (RN) to collaborate with the Clinical Manager and interdisciplinary care team in delivering and coordinating hospice nursing services per each patient’s plan of care. The role emphasizes compassionate care, regulatory compliance, and documentation accuracy.

Responsibilities include comprehensive assessments, care plan development, and effective communication with patients, families, and caregivers.

Formación

• Current, unencumbered RN license eligible for practice in the assigned state(s), including NLC where applicable.
• CPR certification required.
• New graduate RN within the first year of clinical practice.
• Strong communication with patients, families, caregivers of varied ages.
• Interpersonal, verbal, and written communication skills.
• Knowledge of federal/state hospice regulations.
• Valid driver’s license with liability insurance.
• Proficient computer use including email and internet navigation.

Responsabilidades

• Conduct comprehensive patient assessments and develop individualized care plans.
• Manage assigned caseload with care coordination and plan changes.
• Deliver skilled hospice nursing care focused on pain and symptom management, and end-of-life comfort.
• Complete accurate, timely clinical documentation per standards.
• Collaborate with interdisciplinary team to ensure continuity of care.
• Perform other related duties as assigned.
