Enhabit Home Health & Hospice in Oklahoma seeks a licensed RN to coordinate patient care in home-based settings, coordinating with an interdisciplinary team to adapt plans of care to evolving needs.

Candidates must be a graduate of an approved nursing program, hold an RN license in the state, have at least one year of RN experience, and possess CPR certification and basic tech skills. A valid driver's license and reliable transportation are required.

Responsibilities

Provide coordination of care, ensure continuous assessment of each patient's and family's needs, and implement the interdisciplinary care plan. Perform services in accordance with the physician’s orders and the established plan of care (POC). Ensure delivery of care is compliant with the federal and state regulations.

Qualifications

• Must be a graduate of an approved school of nursing education.
• Must be licensed in the state where they currently practice.

Education and experience, preferred

• Medicare home health or hospice experience is preferred.

Requirements

• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions
• Must possess CPR certification for the healthcare provider
