Compassus Home Care and Hospice is seeking a Hospice Registered Nurse to model our values and provide comprehensive care to patients and families in home settings. The RN functions as an IDT member, completing routine and emergency assessments, coordinating care, and educating families while flexing shifts to meet staffing needs, including after-hours on-call responsibilities.

Applicants should hold a nursing degree (associate or bachelor’s) and have at least one year of nursing experience;

Competenze

• Associate or Bachelor's degree in Nursing required.
• Minimum of one (1) year nursing practice or equivalent experience preferred.
• Hospice, oncology, home health and/or long-term care experience preferred.

Mansioni

• Complete routine and emergency assessments on each patient.
• Provide clinical and psycho-social services per plan of care.
• Record observations, treatments, and pertinent information.
• Communicate with IDT, Medical Director, and attending Physician regarding symptom alleviation and responses.
• Provide physical and emotional interventive care to support patient and family during crises.
• Prevent unwarranted hospitalizations by meeting patient and family needs at home.
• Case management of routine and emergency patient care.
• Collaborate with IDT to meet clinical and psycho-social needs and coordinate hospice care.
• Update the plan of care for IDT and notify relevant leaders.
• Educate patient/caregiver on care, disease process, dying process, symptom control, and wound care.
• Submit IDT narratives, time logs, and all required documentation by 10 a.m. the next day.
