Job summary

Rainbow Community Care is seeking a compassionate RN to provide hospice and palliative care, assessing patients and supporting families. The role includes back-up, weekend, and holiday call rotation, with travel to patients' homes and collaboration with the care team.

The RN will assess disease progression, coordinate care, and teach caregivers while maintaining patient comfort, dignity, and confidentiality in a dynamic environment. Strong communication and integrity are essential.

Qualifications

• Active RN license (state-specific)
• Hospice experience preferred
• Valid driver's license and auto insurance

Responsibilities

• Assess initial and ongoing changes in patients' disease progression and impact on needs for adaptive equipment or durable medical equipment.
• Assess caregiver needs and provide teaching and support for patients and families.
• Coordinate with care team to plan and deliver care.
• Ensure infection control practices are maintained in all patient and family interactions.
• Use nursing process to assess care needs and apply to the plan of care.
• Demonstrate flexibility in work schedules and share responsibility for care delivery.
• Use organizational and communication skills to prioritize and supervise daily work for self and team members.
• Document using computerized charting within the shift.
