Job summary

Pathways Healthcare is seeking a dedicated nurse to join our home health and hospice team in Londonderry, NH. You will assess patients, develop and update care plans, and coordinate with physicians to deliver high-quality in-home care.

Responsibilities include on-call duties, supervising staff, and maintaining documentation, with a focus on compassionate, patient-centered care and ongoing professional development.

Qualifications

• 2+ years nursing experience, preferably geriatrics.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver’s license and own a reliable vehicle.

Responsibilities

• Assesses and evaluates patient status and writes plan of care.
• Regularly re-evaluates patient and family/caregiver needs.
• Participates in revising the plan of care as necessary.
• Develops a care plan with goals based on nursing diagnosis.
• Initiates nursing procedures and administers prescribed treatments.
• Completes clinical notes and records changes in pain/symptom management.
• Communicates with physician regarding patient needs and changes in condition.
• Provides and maintains a safe patient environment.
• Attends interdisciplinary group meetings.
• On-call duties and ensures equipment/services are available.
• Supervises ancillary personnel and delegates responsibilities.
