Job summary

Cantex is seeking a Hospice Registered Nurse (RN) to provide skilled nursing care to patients in their homes within the North Houston LV/TD region. You will coordinate with the IDT, physicians, patients, and families to develop and manage hospice plans of care and ensure quality, compliant care.

The role requires strong clinical assessment and independent work in hospice settings, with collaboration across the interdisciplinary team to support end-of-life care.

Qualifications

• Registered Nurse (RN), licensed per state requirements.
• Hospice experience preferred.
• Home Care Home Base (HCHB) experience preferred.
• Strong clinical assessment, communication, and documentation skills.
• Experience working within an interdisciplinary healthcare team.
• Knowledge of hospice regulations, documentation, and care planning preferred.
• Reliable transportation, valid driver's license, and current auto liability insurance.
• Ability to read, write, and communicate effectively in English.

Responsibilities

• Perform initial and ongoing comprehensive nursing assessments for hospice patients.
• Develop, implement, and update individualized hospice plans of care to ensure quality and continuity of care.
• Coordinate patient care with the Interdisciplinary Team (IDT), physicians, patients, and families.
• Educate patients and families on disease process, medications, symptom management, and end-of-life care.
• Determine scope and frequency of nursing services based on patient condition and care needs.
• Supervise and provide clinical direction to Hospice Aides and LVN/LPN staff.
• Maintain accurate and timely clinical documentation in the EMR.
• Communicate changes in patient condition to physicians and IDT.
