Job summary

Hospice of Michigan, Inc. is seeking a registered nurse to provide and supervise hospice nursing services under standard practices. You will perform comprehensive assessments and collab­orate with the interdisciplinary team to tailor patient care plans.

The role emphasizes end-of-life care, family education, and coordination of equipment and supplies, with active participation in quality improvement activities.

Qualifications

• Current RN license to practice in Michigan.
• Hospice experience preferred.
• Ability to use technology in clinical practice.
• Fluent in English with strong communication skills.
• Able to work variable hours including weekends.

Responsibilities

• Perform complete physical assessments and history taking.
• Develop, implement and update the individualized plan of care with IDT input.
• Provide end-of-life care and symptom management per guidelines.
• Educate patients and families on disease, self-care and care goals.
• Coordinate durable medical equipment and supplies for patients.
• Communicate with physicians and team members in a timely manner.
• Supervise Hospice Aides and participate in quality initiatives.
