Job summary

Halifax Health is seeking a Registered Nurse - Hospice to coordinate and implement the care plan for hospice patients to ensure quality of life. Florida RN licensure is required; associate degree in nursing is required, Bachelor's degree a plus.

Qualifications

• Currently licensed as an RN in Florida.
• Associate degree from an accredited School of Nursing.
• Valid driving license and clean driving record.
• Strong interpersonal communication with patients, families, colleagues and physicians.
• Personal professional development, accountability and leadership in nursing practice.
• After-hours and weekend coverage as assigned.
• On-call responsibilities as required by unit.
• Provides care for terminally ill patients and patient/family education.

Responsibilities

• Coordinate and implement the plan of care for hospice patients.
• Provide all aspects of care for terminally ill patients.
• Complete initial and ongoing assessments.
• Provide pain and symptom management.
• Develop an interdisciplinary plan of care reflecting patient/family goals.
• Use critical thinking and participate in Quality Improvement activities.
• Maintain professional relationships and holistic care approach.
