Job summary

Suncrest Hospice in Iowa seeks a Registered Nurse to provide compassionate patient care in home and facility settings, part of an interdisciplinary team. Travel across the service area is required.

As a CHAP-certified hospice, you will support psychosocial assessments, documentation, and coordination with social workers, chaplains, and CNAs, with a focus on high-quality, patient-centered care.

Qualifications

• Current RN licensure (in good standing) in the state(s) of practice.
• Ability to collaborate with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Ability to travel across the service area and manage visits and documentation.

Responsibilities

• Provide care and services for a group of hospice patients in homes, SNFs, and assisted living facilities.
• Serve as a member of the interdisciplinary team with involvement in psych-social assessments and care planning.
• Work in Case Management, Admissions, or Resource roles, as needed.
