Job summary

Heartland Hospice, part of ProMedica, seeks a compassionate Registered Nurse to provide hospice care in home and facility settings. The RN delivers skilled nursing, pain and symptom management, and emotional support to patients with life-limiting illnesses and their families.

Responsibilities include comprehensive assessments, individualized care planning, coordination with the interdisciplinary team, patient and family education, and timely documentation.

Qualifications

• RN license in good standing and eligible to practice in Iowa.
• Experience in hospice or palliative care preferred.
• Strong communication and clinical judgment.

Responsibilities

• Perform comprehensive patient assessments in home and facility settings.
• Develop individualized care plans with the interdisciplinary team.
• Provide pain and symptom management and comfort-focused care.
• Educate patients and families about care goals and therapies.
• Document timely and accurately in medical records.
• Participate in on-call rotation as needed.
• Collaborate with physicians, social workers, and care coordinators.
