Job summary

Heartland Hospice, part of ProMedica, is seeking a compassionate Registered Nurse to provide hospice care in home and facility settings. The RN delivers skilled nursing, pain and symptom management, and emotional support to patients with life-limiting illnesses and their families.

Responsibilities include comprehensive assessments, individualized care planning, coordination with the interdisciplinary team, patient and family education, and timely documentation.

Qualifications

• RN license in good standing
• Hospice care experience preferred
• Strong assessment and communication skills

Responsibilities

• Educate patients and families about care goals and processes
