Job summary

Hartford HealthCare at Home is seeking a registered nurse to deliver skilled nursing care to hospice clients in home and community settings. You will review referrals, perform comprehensive assessments, and collaborate with the interdisciplinary team to develop and modify the plan of care.

You will support patients and families, educate caregivers, and ensure bereavement follow-up where appropriate, while adhering to CT licensing requirements and agency standards.

Qualifications

• Graduate of an ADN program in nursing.
• 1 year nursing experience or one year nursing experience in a specialty.
• License to practice in the state of Connecticut.

Responsibilities

• Review referrals and initiate contact with client and family.
• Conduct complete nursing assessments based on referral data and interviews.
• Develop and modify the plan of care in collaboration with client and disciplines.
• Coordinate care with providers and manage documentation.
• Support bereavement follow-up and teaching to client/family.
