Job summary

AccentCare is seeking a Hospice Registered Nurse (RN) to join our interdisciplinary team in Connecticut. You will provide compassionate, evidence-based nursing care at the center of the patient’s well-being, coordinating with families, physicians, and care colleagues to maintain comfort and dignity in challenging times.

You’ll work Monday through Friday, 8am–5pm, with opportunities for on-call rotations and ongoing professional development within a mission-driven organization.

Qualifications

• RN license in the state of practice.
• Minimum of one year of nursing experience.
• Reliable transportation and valid driver’s license.

Responsibilities

• Provide individualized nursing care that maximizes comfort and quality of life.
• Partner with patients, families, physicians, and the interdisciplinary group to create and carry out care plans.
• Perform comprehensive assessments - physical, emotional, social, and spiritual to guide care decisions.
• Educate and support patients and caregivers, meeting them where they are.
• Coordinate care across disciplines to ensure continuity and dignity.
• Document care clearly and accurately to reflect patient needs and progress.
• Participate in on-call or admission rotations as needed.
• Engage in ongoing learning and professional development.
