Job summary

Three Oaks Hospice is hiring skilled registered nurses to provide in-home end-of-life care, coordinating with the interdisciplinary team and families to deliver compassionate, high-quality care.

Responsibilities include comprehensive assessments, implementing care plans, patient/family education, and on-call coverage. Candidates must hold an active RN license, CPR, and 2 years of clinical RN experience, with hospice or home health preferred.

Qualifications

• Registered nurse with an Associate or Bachelor’s degree in Nursing.
• Current state RN license and valid driver’s license.
• Two years’ RN experience in a clinical care setting; hospice/home health preferred.
• Current CPR certification.
• Proficient with computers; Microsoft Office suite experience preferred.
• Strong interpersonal, communication, and organizational skills with high ethics and compliance.

Responsibilities

• Administer skilled nursing care in patients’ homes or residences.
• Coordinate care with the interdisciplinary team, patient/family, and referring agency.
• Perform comprehensive assessments and implement the Plan of Care (POC).
• Educate patients and families about disease process, self-care, end-of-life care, nutrition and dietary needs.
• Initiate preventive and rehabilitative nursing procedures and document progress.
• Coordinate services and prioritize needs with IDT; identify additional referrals as needed.
• Communicate changes in patient needs to physicians and team; monitor outcomes.
• Determine scope and frequency of services based on acuity and family needs.
• Handle on-call responsibilities and provide on-call service as required.
• Perform other duties as assigned.
