Job summary

Interim HealthCare is seeking a dedicated Hospice RN Case Manager to serve patients in Charleston, Berkeley, and Dorchester Counties. You will lead comprehensive assessments, develop care plans with the IDG, and coordinate services to support patients and families in a home-based hospice setting.

Responsibilities include on-call participation, ongoing status updates, and supervising LPN/LVN, hospice aides, and homemakers while ensuring the highest standards of nursing practice and compassionate

Qualifications

• Must be a graduate of an accredited school of nursing.
• Active RN license in South Carolina in good standing.
• 12 months RN experience within last 5 years; or current clinical knowledge as deemed appropriate.
• New graduates: 1200 verified hours or completion of a Preceptor Program.
• 1 year of hospice care experience preferred.
• 1 year of precepting or training experience preferred.
• CPR certification required.
• Valid driver’s license and auto liability insurance.

Responsibilities

• Maintains a specified case management load.
• Completes an initial/comprehensive patient/family assessment to determine hospice needs and pain/symptom management.
• Participates in on-call duties per office policy.
• Performs ongoing assessments of patient and family status.
• Initiates and develops an IDG care plan with patient/family and attending physician.
• Revises the IDG plan as patient/family needs change.
• Coordinates care among IDG members and external providers.
• Evaluates and coordinates need for services per the IDG plan.
• Directs services and supervises LPN/LVN, Hospice Aide, and Homemaker.
• Facilitates consideration of higher levels of care when indicated.
• Handles PHI in accordance with organizational policies.
