Job summary

Interim HealthCare is seeking a dedicated Hospice RN Case Manager to lead compassionate, patient-centered care across Charleston, Berkeley, and Dorchester Counties.

You will assess patients, develop and coordinate the IDG plan of care, supervise the care team, and support families while maintaining high nursing standards. This full-time role offers competitive pay, weekly pay, and a strong benefits package.

Qualifications

• Graduate of an accredited school of nursing.
• Licensed to practice as a RN in the state of South Carolina.
• Twelve (12) months of RN experience in the last five years, or current clinical knowledge if no recent experience.
• One year of hospice care experience preferred.
• CPR certification.
• Able to provide proof of valid driver’s license and auto liability insurance.

Responsibilities

• Maintains a specified case management load.
• Completes initial/comprehensive assessment of patient/family to determine hospice needs.
• Participates in on-call duties as defined by the office on-call policy.
• Completes ongoing assessments of patient’s physical and psychosocial status.
• Initiates and develops a plan of care with IDG, patient/family, and attending physician.
• Makes revisions to IDG plan as patient status changes.
• Coordinates care among IDG members and other healthcare providers outside IDG.
• Evaluates and coordinates need for services per IDG plan.
• Directs services and supervises LPN/LVN, Hospice Aide, and Homemaker.
• Facilitates consideration of higher or different levels of care when indicated.
