Job summary
Community Hospice is seeking a Hospice Registered Nurse to join our Nursing Home Care Team in the Capital Region (Albany area). This full-time day shift role supports patients and families in local nursing homes, with no weekends and no on-call.
You will manage hospice patients, coordinate with nursing home staff, and provide symptom management and emotional support. A NY RN license, ADN or BSN, and LTC experience are required; hospice experience is a plus.
Qualifications
- Current license to practice as a Registered Nurse in New York State.
- Associates Degree in Nursing; BSN preferred.
- 1 year experience as an RN required; long-term care experience required.
- Hospice experience preferred but not required.
- Valid Driver's License and reliable vehicle.
- Ability to communicate effectively, verbally and in writing.
- Strong interpersonal skills to build relationships with patients, families, physicians and coworkers.
Responsibilities
- Case manage patients in nursing home settings and respond to symptom management needs; coordinate with the team for resident and family support.
- Collaborate with nursing home staff in hospice patient care.
