Legacy Hospice is seeking a compassionate Registered Nurse Case Manager (RNCM) to join our mission-driven team in Amory, MS. In this home-based role, you’ll deliver high-quality end-of-life care while supporting families with empathy and respect.

You will assess patients, manage a caseload with the interdisciplinary team, administer medications per orders, educate families, and maintain clear clinical documentation. A BSN or CHPN is preferred and CPR/BLS certification is required.

Qualifications

• RN licensed in the state of employment.
• BSN or CHPN preferred.
• Minimum 1 year of nursing experience in hospice, home health, or related field.
• Active CPR/BLS certification.
• Valid driver’s license and reliable transportation.

Responsabilités

• Perform comprehensive patient assessments and create individualized care plans.
• Manage a caseload of hospice patients, coordinating care with the IDT.
• Administer medications and treatments per physician orders.
• Educate patients and families on symptom management and end-of-life care.
• Monitor and update care plans based on ongoing assessments.
• Provide emotional and clinical support to patients and caregivers.
• Communicate with physicians, care partners, and community resources.
• Maintain accurate, timely clinical documentation.
• Participate in IDT meetings and on-call rotation.
• Supervise ancillary staff and ensure continuity of care.
