Ascend Hospice in Lawrence, KS seeks an RN Case Manager to support on-call coverage and case management. You will perform patient assessments, develop care plans, and coordinate with the healthcare team to ensure continuous, well-coordinated care. Travel is required and a valid RN license is mandatory.

Qualifications

• State-licensed RN with active license.
• At least 2 years nursing experience; hospice preferred.
• Willingness to travel and valid driver's license.

Responsibilities

• Complete initial patient/family assessment to determine hospice needs.
• Regularly re-evaluate patient nursing needs and adjust care plan.
• Collaborate with healthcare team to coordinate continuous care.
• Counsel patient and family on end-of-life needs.
• Initiate appropriate preventive and palliative nursing procedures.
• Communicate with physician about patient changes and needs.
• Supervise Hospice Aides.
