Job summary

Qualifications

Responsibilities

• Conduct initial assessments of patients and families to determine hospice care needs.
• Regularly re-evaluate patients' nursing needs and adjust care plans.
• Use data to determine care plan requirements.
• Collaborate with the healthcare team, patient, and family to coordinate care.
• Counsel patients and families on end-of-life needs.
• Initiate appropriate preventive and palliative nursing procedures.
• Communicate with physicians about patient needs and changes in condition.
• Supervise hospice aides.
