Ascend Hospice & Palliative Care PA in York, PA is hiring an RN Case Manager to join our compassionate team. You will perform assessments, develop care plans, and coordinate with physicians and the care team to deliver high-quality end-of-life care.

The role requires an active RN license, 2 years nursing experience (hospice preferred), a valid driver's license, and willingness to travel. We offer a $7,500 sign-on bonus, competitive salaries, and robust benefits. Hours are Mon-Fri 8AM-5:30PM.

Qualifications

• Must have a State Certified RN license.
• 2 years of professional nursing experience; hospice experience preferred.
• Valid driver's license and reliable transportation.
• Willingness to travel as required.
• Strong verbal and written communication skills.

Responsibilities

• Complete initial patient and family assessment to determine hospice needs.
• Regularly re-evaluate patient nursing needs and adjust care plans.
• Use assessment data to determine care plan needs.
• Collaborate with health team, patient, and family for coordinated care.
• Counsel patient and family on end-of-life needs.
• Initiate preventive and palliative nursing procedures as appropriate.
• Communicate with physicians about patient needs and status changes.
• Supervise Hospice Aides.
