Job summary

Agrace in Wisconsin is seeking a Nurse Case Manager (RN Case Manager) to deliver patient- and family-centered care through assessments, planning, and coordination of services. This full-time role includes day shifts, on-call rotations, and driving as needed across the Madison area.

You will work with the interdisciplinary team to develop and revise the plan of care, document in the EHR, and support hospice goals while mentoring staff and guiding care transitions.

Qualifications

• ADN or BSN required.
• Current, unrestricted RN license in the state of employment.
• One-year nursing experience preferred.
• Valid driver’s license with access to reliable vehicle.
• Must be at least 18 years old.
• Basic Microsoft Office skills.
• Intermediate electronic health records skills.
• Ability to use proper body mechanics and safe patient handling.

Responsibilities

• Handle patient and family assessments per plan of care.
• Provide patient visits, interventions, and pain management.
• Develop, implement, and revise the plan of care with the Interdisciplinary Team.
• Delegate visits and oversee CNAs/LPNs.
• Participate in meetings and precept new staff.
