Job summary

Mission Healthcare, a leading home health and hospice provider across the western United States, is seeking an experienced Registered Nurse for the Carmel Valley to Carlsbad area. You will assess patients in home settings, develop and revise plans of care, and coordinate with families and physicians to deliver compassionate in-home care.

The ideal candidate holds an active RN license, at least 1 year in acute care, BLS, and a valid driver's license.

Qualifications

• Active RN license in the state where you practice.
• Minimum 1 year of recent acute care experience in an institutional setting.
• BLS/CPR certification is required.
• Valid driver's license and auto insurance.

Responsibilities

• Completes initial patient and family assessment to determine home care needs.
• Initiates and revises the plan of care as patient status changes.
• Regularly re-evaluates patient needs and adjusts care.
• Uses assessment data to determine nursing diagnoses.
• Develops care plans with goals based on diagnosis and family involvement.
• Administers medications and treatments as prescribed.
• Counsels patients and families on plan of care and needs.
• Provides health care instructions per assessment and plan.
• Identifies discharge needs and supports planning before discharge.

Pay range (depending on experience): $55.00 - $60.00 hourly + $5K Sign On Bonus!

Monday - Friday, 8:00am - 5:00pm

Territory/Location

Carmel Valley to Carlsbad area

Responsibilities

• Completes an initial assessment of the patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Regularly re-evaluates patients’ needs.
• Uses health assessment data to determine nursing diagnosis.
• Develops a care plan which establishes goals, based on nursing diagnosis, incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.
• Counsels the patient and family in meeting plan of care and related needs.
• Provides health care instructions to the patient as appropriate per assessment and plan.
• Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.

Qualifications

• Current and valid state RN license.
• Minimum of 1 year of recent acute care experience in an institutional setting.
• Basic Life Support (BLS), with hands-on CPR certification.
• Valid driver's license and auto insurance.
• Bachelor’s degree in Nursing, and 1 year of home health experience preferred.
