Job summary

Mission Healthcare is hiring an RN for full-time home health care in the Greater Riverside/San Bernardino area. The role involves assessing patients, initiating and updating care plans, and delivering nursing care in the home setting.

You will provide patient education, administer medications, and coordinate discharge planning with families and physicians. A valid RN license and BLS are required; 1 year of acute or home health experience is preferred.

Qualifications

• Current and valid state RN license.
• Bachelor's degree in Nursing; 1 year of home health experience preferred.
• Basic Life Support (BLS) certification required.

Responsibilities

• Completes initial patient and family assessment to determine home care needs.
• Develops and revises the plan of care based on patient status and needs.
• Regularly reevaluates patient needs and status.
• Administers medications and treatments as prescribed by the physician.
• Counsels patient and family on plan of care and health instructions.
• Identifies discharge planning needs as part of care planning.

Pay range (depending on experience): $55.00-$63.00 hourly

Schedule/Shift : Full Time, Monday-Friday: 8:00am-5:00pm

Territory/Location: Greater Riverside/San Bernardino,CA

Responsibilities:

• Completes an initial assessment of the patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Regularly re-evaluates patients' needs.
• Uses health assessment data to determine nursing diagnosis.
• Develops a care plan which establishes goals, based on nursing diagnosis, incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.
• Counsels the patient and family in meeting plan of care and related needs.
• Provides health care instructions to the patient as appropriate per assessment and plan.
• Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.

Qualifications:

• Current and valid state RN license.
• Minimum of 1 year of recent acute care experience in an institutional setting.
• Basic Life Support (BLS), with hands-on CPR certification.
• Valid driver's license and auto insurance.
• Bachelor's degree in Nursing, and 1 year of home health experience preferred.
