Job summary

Optimal Care, a clinician-owned provider in Jackson, MI, is seeking a Hospice RN Case Manager to deliver skilled nursing care with compassion for patients and families in their homes and facilities.

You will coordinate care across a multidisciplinary team, build meaningful relationships, and autonomously manage a caseload while addressing physical, emotional, and spiritual needs. This role requires travel within the Jackson area and supports a comprehensive care approach.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State License as a Registered Nurse.
• Minimum one year experience as a nurse in a clinical care setting.
• Reliable transportation, valid driver’s license, and valid automobile insurance coverage.

Responsibilities

About the Role

Bring your nursing expertise to one of healthcare's most meaningful specialties. As a Hospice RN Case Manager, you'll provide skilled nursing care and compassionate support to patients and families during life's final chapter, helping ensure comfort, dignity, and quality of life.

This role is perfect for a registered nurse who wants to practice the full scope of nursing while building deep, meaningful relationships. This position will have you building report with patients, families, physicians, facilities, and other interdisciplinary team members. You'll have the autonomy to coordinate comprehensive care for your caseload while working collaboratively with our interdisciplinary team to address the physical, emotional, and spiritual needs of those in your care.

Location

• Office Location: Jackson, MI
• Main Service Area: Jackson Area
• This role requires daily travel within the service area to provide care to patients in their homes, assisted living facilities, or nursing homes

Hours

• Monday to Friday, 8 a.m. to 5 p.m. - TBD
• Weekend On-Call Rotation

Required Qualification

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing

• Minimum one (1) year experience as a nurse in a clinical care setting
• Reliable transportation, valid driver’s license, and valid automobile insurance coverage

Preferred Qualifications

• Bachelor of Science in Nursing (BSN)
• Hospice or home health experience

• Demonstrates ability to assess and respond to the needs of patients, families and caregivers in varied settings and to function effectively in the home or nursing facility environment
• Demonstrates skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles
• Ability to handle multiple priorities, documentation requirements and deadlines

Pay Range

$75,000 - $82,000 USD
