Job summary

St. Croix Hospice in Wisconsin is seeking a Registered Nurse to provide day-to-day hospice care under the guidance of an RN Case Manager, focusing on comfort and quality of life for patients and families in their homes.

You will administer medications, document care, participate in Plan of Care revisions, and collaborate with an interdisciplinary team to meet emotional and physical needs while maintaining HIPAA compliance.

Qualifications

• Graduate of an accredited school of nursing required.
• Must have a current Registered Nurse (RN) license in state(s) of practice.
• Ability to pass background, consumer report and drug test.
• Valid State Driver’s License, access to an automobile in good working order, and driving record required.

Responsibilities

• Provides day-to-day hospice patient care under the direction of the patient’s RN Case Manager with a focus on comfort and quality of life.
• Administers medications and maintains accurate clinical notes.
• Writes and initiates Plan of Care and participates in Plan of Care revisions.
• Coordinates Plan of Care with relevant community health partners and ensures necessary equipment and services are available.
• Maintains HIPAA compliance and protects patient confidentiality.
• Works with the interdisciplinary care team to address emotional needs of the patient and family/caregiver.
