Job summary

Kindful Health is seeking an RN Case Manager to lead a care team and provide best-in-class hospice care for patients and families in Texas. You will collaborate with LPNs, aides, MSW and a chaplain to manage a reasonably sized census and ensure coordinated care.

You will present biweekly at IDT and work with the Clinical Director and Medical Director to maintain high standards and a concierge hospice experience. Texas RN license and hospice experience required.

Qualifications

• Must be a graduate of an approved school of professional nursing.
• Must have at least 2 years of nursing experience in the Hospice service line in Texas.

Responsibilities

• Adheres to all regulatory requirements and professional standards.
• Follows policies and procedures consistent with recognized standards of professional practice.
• Identifies system problems or inefficiencies that may negatively affect patient care.
• Manages clinical excellence within your patient census.
• Provides high levels of communication with the Kindful team and with patients and families to ensure top CAHPS scores.
• Collaborates with Clinical Director, Medical Director and the team to deliver a concierge level hospice experience.
