Job summary

Community Healthcare of Texas is seeking an Inpatient RN to provide compassionate hospice and palliative nursing care in Burleson, TX. The role includes comprehensive patient assessments, care planning, and coordinating with the interdisciplinary group to meet emotional and physical needs.

The ideal candidate holds a Texas RN license with at least one year of nursing experience, preferably in hospice or home health, and strong computer skills to document care and communicate with the care team.

Qualifications

• Licensed as a Registered Nurse by the State of Texas.
• Minimum one year nursing experience.
• Hospice/Home Health experience preferred.
• Proficient computer skills; MS Office familiarity.
• Ability to work independently and manage multiple patients.

Responsibilities

• Perform comprehensive patient assessments and plan of care.
• Provide emotional support for patients and families in end-of-life care.
• Develop and revise care plans based on nursing diagnosis.
• Case manage total plan of care and coordinate with the team.
• Maintain accurate clinical notes and document changes.
• Administer medications and treatments as prescribed by the physician.
• Coordinate with the IDG and discharge planning teams.
