Job summary

St. Paul Elder Services is seeking a compassionate Hospice RN Case Manager to join our team in Kaukauna, WI. This part-time role involves providing person-centered end-of-life care, conducting assessments, and coordinating with an interdisciplinary care team to support patients and families.

The RN will work primarily day shifts with on-call rotation every third week, ensuring comfort and dignity for patients across our service area. Hospice experience is preferred but not required.

Qualifications

• Current RN license required.
• Hospice experience preferred but not required.
• Experience in palliative care or services for older adults is highly desirable.
• Strong clinical assessment, communication, and care coordination skills.

Responsibilities

• Provide individualized nursing assessments and develop patient-centered plans of care.
• Support patients receiving hospice services at Kaukauna and Green Bay campuses and throughout the five-county service territory.
