Job summary

Affinity Hospice is seeking an experienced Registered Nurse (RN) to join as a hospice case manager. You will plan, organize, and direct hospice care in patients’ homes, coordinating with an interdisciplinary team to meet patient and family needs.

The role requires an active RN license, CPR, and at least two years in clinical care, home health, or hospice. On-call duties include weekends; driving to patient homes is required, with lifting and movement assistance as needed.

Qualifications

• Graduate of an accredited nursing program (Diploma/ADN/BSN).
• Current State license as a Registered Nurse (RN).
• Current CPR certification.

Responsibilities

• Case manages and directs the interdisciplinary team, physician, and family.
• Implement and document patient plan of care.
• Provide care with infection control measures to protect staff and patients.
• Ensure continuity of quality patient care with proper documentation.
• Monitor cases for third-party payer requirements.
• On-call responsibilities and service to patients/families as assigned.
• Supervise hospice aides and licensed practical nurses.
