Job summary

St. Croix Hospice in Eagan, MN is seeking an experienced RN Case Manager to guide patients and families through the end-of-life journey with compassionate care.

You will assess needs, develop and revise the Plan of Care, administer medications, educate families, document thoroughly, coordinate with physicians and care teams, and ensure HIPAA compliance in home settings. This role requires participating in on-call visits, collaborating with interdisciplinary teams, and ongoing professional

Qualifications

• Must have a current Registered Nurse (RN) license in state of practice.
• Multistate license holders may practice in other states while maintaining RN license in primary state.
• Ability to pass DHS background, consumer report, and drug test.
• Valid State Driver’s License and automobile in good working order.

Responsibilities

• Provides care in accordance with the Nurse Practice Act and nursing standards.
• Conducts comprehensive patient and family assessments to determine hospice needs.
• Develops, implements, and revises the Plan of Care based on ongoing evaluations.
• Administers medications and treatments as prescribed.
• Educates patients and families on self-care, medications, and diet as appropriate.
• Maintains accurate clinical documentation of pain, symptoms, and condition changes.
• Coordinates care with physicians, interdisciplinary teams, and community partners.
• Ensures a safe environment and assists patients with daily living activities.
• Provides emotional support and counseling to patients and families.
• Arranges for medical equipment and supplies as needed.
• Ensures HIPAA compliance and patient confidentiality.
