Job summary

Caritas Hospice is seeking an experienced RN Case Manager to join our compassionate team in Texas. You will perform initial and ongoing assessments of patients facing life-limiting illness, develop and adjust care plans, and collaborate with physicians, social workers, and other specialists to coordinate comprehensive home or facility-based hospice care.

The ideal candidate holds an accredited nursing degree, an active Texas RN license, a valid driver’s license, and at least two years of nursing

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.
• Two years of experience as an RN preferred in a healthcare/clinical setting.
• Current Driver's License and reliable transportation.

Responsibilities

• Initial and ongoing comprehensive assessments of the impact of terminal disease on patients’ physical, psychosocial, and environmental needs and ADLs.
• Implement the individualized POC and revisions to the plan as necessary.
• Collaborates care amongst patient/patient caregiver, physicians and healthcare professionals.
• Attend Inter-Disciplinary Group (IDG) meeting and coordinate in care planning with IDG.
• Perform on-call responsibilities and provide on-call service to patients/families as assigned.
• Timely submission of all documentation.
• Performance of other duties as required.
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures.

What You Must Have:

• Attend Inter-Disciplinary Group (IDG) meeting, coordinate and participate in the patient care planning process, prioritization of needs and collaborates with IDG to promote coordination of patient care


• Perform on-call responsibilities and provide on-call service to patients/families as assigned


• Timely submission of all documentation


• Performance of other duties as required
