POSITION OVERVIEW

The Hospice Registered Nurse Case Manager provides skilled nursing care and comprehensive case management services to hospice patients throughout Denton County.

In this role, you will assess patient needs, manage pain and symptoms, coordinate individualized plans of care, and provide education and emotional support to patients and families throughout the hospice journey. As the primary clinical coordinator for your assigned caseload, you will work closely with physicians and the interdisciplinary team to ensure every patient receives compassionate, patient-centered care.

KEY RESPONSIBILITIES

• Provide skilled, compassionate nursing care in accordance with each patient's individualized plan of care.
  
• Perform comprehensive patient assessments and continuously monitor changes in condition.
  
• Coordinate and manage the clinical care of assigned hospice patients.
  
• Administer medications and treatments as prescribed by the physician.
  
• Assess, manage, and monitor pain and other distressing symptoms.
  
• Educate patients, families, and caregivers regarding medications, symptom management, safety, and the hospice philosophy of care.
  
• Collaborate closely with physicians and the interdisciplinary hospice team to ensure coordinated, patient-centered care.
  
• Communicate significant changes in patient condition promptly and appropriately.
  
• Respond to urgent patient needs and provide crisis intervention when necessary.
  
• Maintain complete, accurate, and timely clinical documentation in accordance with company policies, Medicare regulations, and all applicable standards.
  
• Participate in the rotating on-call schedule.
  
• Travel throughout the assigned Denton County service area.
  

IDEAL CANDIDATE

The successful candidate will possess:

• Minimum of one year of hospice nursing experience.

REQUIRED QUALIFICATIONS

• Current, unrestricted Texas Registered Nurse license.
  
• Minimum of one year of hospice nursing experience.
  
• Valid driver's license.
  
• Ability to travel throughout Denton County.
  
• Ability to participate in the rotating on-call schedule.
  

SCHEDULE

• Full-time
  
• Monday through Friday
  
• Rotating on-call responsibilities
  
• Local travel throughout the assigned Denton County territory
  

COMPENSATION

• Base Salary: $85,000–$90,000 annually, based on experience and qualifications
  
• Mileage Reimbursement: $0.65 per mile
  
• Cell Phone Reimbursement: $60 per month
