Job summary

Superior Hospice is seeking a Hospice RN Case Manager to provide one-on-one patient care across the Greater Twin Cities Metro Area. You will manage a personal caseload, conduct assessments, and develop individualized plans to optimize comfort and quality of life.

You will work with an interdisciplinary team, maintain EMR documentation, and participate in weekend/on-call rotations while delivering compassionate hospice care to families.

Qualifications

• Active Minnesota RN license required.
• Hospice experience preferred or related complex care background.
• Strong clinical judgment and ability to work independently.

Responsibilities

• Manage your own hospice caseload as primary RN for patients and families.
• Complete comprehensive nursing assessments and develop individualized plans of care.
• Provide expert pain and symptom management focused on comfort and dignity.
• Guide and educate patients, caregivers, and families throughout the hospice journey.
• Coordinate care with physicians, nurse practitioners, social workers, chaplains, aides, therapists, and partners.
• Provide care in patients' homes, assisted living, and skilled nursing facilities.
• Maintain accurate EMR documentation and comply with Medicare hospice regulations.
• Participate in a weekend and on-call rotation for continuity of care.
