Job summary

Superior Hospice is seeking a Hospice RN Case Manager to become the nurse patients remember. You’ll manage a personal caseload and deliver one-on-one hospice care in patients’ homes and facilities.

In this role you’ll build meaningful relationships, collaborate with an interdisciplinary team, and provide pain and symptom management to improve quality of life across a 21-county service area in the Greater Twin Cities Metro Area.

Qualifications

• Active Minnesota RN license/registration.
• Hospice experience preferred; oncology/ICU/palliative or home health background acceptable.
• Strong clinical judgment and ability to work independently.
• Excellent communication and relationship-building skills.
• Willingness to travel within assigned territory.

Responsibilities

• Manage an assigned hospice caseload across the continuum of care.
• Provide pain and symptom management focused on comfort and quality of life.
• Serve as the primary Hospice RN Case Manager for patients and families.
• Complete comprehensive nursing assessments and individualized care plans.
• Educate and support caregivers throughout the hospice journey.
• Collaborate with physicians, nurse practitioners, social workers, chaplains, hospice aides, therapists, and community partners.
• Provide care in patients' homes, assisted living communities, and skilled nursing facilities.
• Maintain accurate EMR documentation and participate in a weekend and on-call rotation.
