Job summary

Agrace is seeking a skilled RN Case Manager (Nurse Case Manager) for our Janesville, WI location. This full-time day-shift role includes on-call responsibilities (one weekend per quarter), weekend and holiday rotations, and a $6,000 sign-on bonus.

You will assess patients, develop and update care plans, and provide education in alignment with Agrace philosophy and policies. Responsibilities include patient/family assessments, planning and coordinating care with the IDT, and supervising care

Qualifications

• Active RN license in the state of employment.
• BSN or ADN required.
• One year nursing experience preferred.
• Basic Microsoft Office skills and intermediate EHR experience.

Responsibilities

• Assess patient and family needs per the plan of care.
• Provide nursing interventions, pain management, and education.
• Develop, implement, and revise individualized plan of care with IDT.
• Document interventions, outcomes, and changes in the EHR.
• Oversee nursing staff and participate in meetings; precept new staff.
