Job summary

Amedisys is seeking an RN Hospice Case Manager to join our team in North Carolina. You will perform patient assessments, collaborate with the care team, and develop individualized plans of care to support patients at home.

Responsibilities include on-call coverage, documenting care progress, and teaching patients and families. A valid RN license, CPR certification, and a reliable vehicle are required; compensation is $82,000–$86,000 with full benefits.

Qualifications

• One year of nursing experience.
• Current RN license valid in the assigned state(s).
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Performs patient assessments and collaborates with the care team.
• Develops and maintains individualized patient care progress records.
• Records pain/symptom changes and outcomes.
• Teaches patients, families, and caregivers appropriate care techniques.
• Communicates regularly with the clinical manager and care team.
• Performs on-call responsibilities as assigned.
• Other duties as assigned.
