Job summary

Serenity Hospice is seeking a Registered Nurse (RN) Case Manager to join our Lancaster, PA team. You will coordinate and manage hospice care for assigned patients, conducting assessments and creating personalized care plans in collaboration with the hospice team.

Qualifications

• Active Pennsylvania RN license.
• Hospice or home health experience preferred.
• Excellent communication and organizational skills.
• Compassionate, patient-centered approach.
• Valid driver’s license and reliable transportation.

Responsibilities

• Coordinate and manage hospice care for your assigned patients.
• Perform comprehensive patient assessments and develop individualized care plans.
• Collaborate with physicians and the interdisciplinary hospice team.
• Provide compassionate education, guidance, and support to patients and families.
• Maintain accurate and timely documentation in accordance with hospice regulations.
• Build meaningful relationships with patients and families during an important time in their lives.
