Job summary

AMEDISYS HOME HEALTH is seeking an experienced RN case manager in Massachusetts to lead patient assessments, coordinate care plans, and collaborate with the care team to ensure high-quality home health services. The role requires an active RN license and CPR certification, with reliable transportation for patient visits.

The position offers a full-time, day-shift schedule with territory coverage in Greater Framingham and surrounding areas, orientation in Marlborough, and a comprehensive benefits

Qualifications

• One year of nursing experience.
• Current RN license for the applicable state.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Develops, prepares, and maintains individualized patient care progress records with accuracy and timeliness.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
