Job summary

UnitedHealth Group's Assured Hospice of Olympia in Olympia, WA seeks an experienced Registered Nurse Case Manager to lead the patient’s Plan of Care in home hospice settings. You will collaborate with families, physicians, and the inter-disciplinary group to promote comfort, dignity, and quality of life.

Responsibilities include initial evaluation within 48 hours, ongoing reassessments, care coordination, and precise documentation while supporting patients and families through the dying process.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• 1+ years of clinical nursing experience.
• Current CPR certification.
• Valid driver''s license, vehicle insurance, and reliable transportation.

Responsibilities

• Perform initial nursing evaluation within 48 hours of referral.
• Identify patient/family needs and reassess at least every 15 days.
• Initiate and coordinate the plan of care.
• Document problems, goals, interventions, and responses.
• Collaborate with patient/family, attending physician and IDG daily.
• Instruct and supervise patient/family in self-care when appropriate.
• Maintain accurate clinical notes on patient condition.
