Job summary

Beth Israel Lahey Health in Massachusetts is seeking a Registered Nurse for Hospice care across home and facility settings. You will develop patient-centered plans of care, collaborate with an interdisciplinary team, and provide expert pain and symptom management for terminally ill patients.

The role requires current MA RN license, hospice case management experience, competency in PTCT EMR, and strong communication with patients, families, and healthcare colleagues.

Qualifications

Current Massachusetts professional RN license.
Recent Hospice Case Management or related Home Health Experience.
Expertise in pain and symptom management for end-of-life care.
Ability to work in an interdisciplinary team with patients and families.

Responsibilities

Provides hospice care per philosophy and patient plan of care.
Acts as eyes and ears of the physician in patient settings and communicates assessments.
Performs RN pronouncement in all settings except acute care hospital.
Maintains and updates patient care plans every 14 days and at recertification.
Coordinates care with physicians, pharmacists, and IDG for optimal end-of-life care.
Educates patient, family, and contracted caregivers on medications and care processes.
