Job summary

The Elizabeth Hospice is seeking an RN Case Manager to provide professional nursing care for adults, pediatric and perinatal patients in the San Diego area. Working with the IDT, you assess, plan, implement and evaluate the plan of care while coordinating with internal and external partners.

This field-based role requires travel across North San Diego County and South Riverside County, a CA RN license, CPR, and driving.

Qualifications

• Must have Associate's degree plus one (1) year of nursing experience within the past three (3) years or a BSN from ACEN-accredited program.

Responsibilities

• Develops written plan of care in coordination with the IDT.
• Oversees implementation and evaluates the plan to meet patient/family needs.
• Directs care provided by LVN and HA according to the plan of care.
