Job summary

Moments Hospice is seeking an experienced RN Case Manager to lead clinical care for patients and families at the end of life. You will assess needs, coordinate services, and manage symptoms within a supportive interdisciplinary team.

This field-based role offers a Monday–Friday schedule, a competitive salary of $75,000–$90,000, and a fleet car program with coverage for gas and insurance to support day-to-day patient care.

Qualifications

• Active RN license in the state of work.
• Experience in hospice or palliative care is preferred.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients’ physical, emotional, and psychosocial needs
• Develop, implement, and update individualized plan of care in partnership with patients and families
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures
• Educate patients and caregivers on disease progression, symptom management, medications, and what to expect at end of life
• Coordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care team
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care
