Moments Hospice is seeking an RN Case Manager to lead patient care in a field-based role. You will assess needs, create and update care plans, and deliver skilled nursing care while educating families on end-of-life care. You’ll coordinate with physicians and the care team to ensure compassionate, holistic support.

Qualifikationen

• Active RN license in the state of work.
• Proficient in electronic medical records (EMR).
• Strong organizational, communication, and critical-thinking skills.
• Ability to work independently while maintaining high standards of care.
• Compassionate, patient- and family-centered approach.

Aufgaben

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs.
• Develop, implement, and update individualized plan of care in partnership with patients and families.
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures.
• Educate patients and caregivers on disease progression, symptom management, medications, and what to expect at end of life.
• Coordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care team.
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care.
