Day City Hospice in Dayton, OH is seeking an experienced Registered Nurse Case Manager to guide patients and families through the hospice journey. You will serve as the primary clinical resource, coordinating assessments, care planning, and ongoing support with an emphasis on compassionate, patient-centered care.

Qualifications

• Graduate of an accredited Registered Nursing program.
• Current RN license in Ohio.
• CPR and First Aid certification.
• Knowledge of quality assessment and performance improvement practices.
• Valid driver's license and reliable transportation.

Responsibilities

• Manage and coordinate patient and family caseloads in hospice care.
• Conduct comprehensive assessments and determine care needs.
• Develop, implement, and update individualized care plans.
• Collaborate with physicians and the interdisciplinary team to ensure quality outcomes.
• Identify physical, emotional, social, and spiritual needs; coordinate interventions.
• Provide regular patient visits and reassess care as conditions change.
• Educate, support, and counsel patients and families.
