Job summary

Hospice of the Chesapeakes is seeking RN Case Managers to provide nursing care and case management for patients with life-limiting illnesses in their homes in Charles County, Maryland. This full-time role focuses on care continuity and quality, working with an interdisciplinary team including social workers, chaplains, CNAs, and hospice physicians.

Responsibilities include assessing patients during visits, leading the journey through illness, and coordinating goals with caregivers.

Qualifications

• Current Maryland RN license or accepted reciprocal out-of-state nursing licensure; BSN preferred.
• BSN preferred as a higher qualification (education).
• 2+ years of nursing experience in med/surg or home care; hospice/end-of-life/critical care preferred.

Responsibilities

• Engage with patients to assess physical, psychosocial, and emotional condition during regular nursing visits.
• Lead the patient journey and ensure understanding of each step by patients and families.
• Collaborate with the team to establish goals of care and keep all parties updated on progress.
