Job summary

Moments Hospice is seeking an experienced RN Case Manager to lead clinical care for patients and families in a field-based role. You will assess needs, coordinate services, and manage symptoms to ensure comfort and dignity in the final chapter of life.

You will work with a supportive team, with reasonable caseloads and direct access to leadership. A fleet car program covers business and personal use, enhancing focus on patient care.

Qualifications

• Active Registered Nurse (RN) license in the state you work in.
• Proficiency with electronic medical records (EMR).
• Strong organizational, communication, and critical-thinking skills.
• Ability to work independently while maintaining high standards of care.
• A compassionate, patient- and family-centered approach.
• Valid driver’s license and reliable vehicle with current insurance.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients’ physical, emotional, and psychosocial needs.
• Develop, implement, and update individualized plan of care in partnership with patients and families.
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures.
• Educate patients and caregivers on disease progression, symptom management, medications, and what to expect at end of life.
• Coordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care team.
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care.
