Job summary

Kamin Science Center is seeking an RN Case Manager to plan and coordinate nursing care for an assigned caseload across home and long-term care settings. You will educate patients and families, collaborate with the care team, and ensure goals are met using a holistic approach.

Flexibility in work location across care settings is required. Must have Florida RN license and CPR, with at least 1 year of RN experience; hospital med-surg/oncology preferred.

Qualifications

• Registered Nurse in Florida with current license.
• Minimum 1 year RN experience; hospital med-surg/oncology preferred.
• CPR certification and valid driver’s license required.

Responsibilities

• Assess patient and family goals and needs; develop care plans.
• Educate patients and caregivers about disease progression and hospice care.
• Coordinate timely responses to changing patient needs.
• Document accurately and participate in quality improvement.
