Job summary

VNA Care is seeking a dedicated Hospice Registered Nurse to serve as Case Manager in our home care program across Marlboro, Framingham, Holliston and Franklin. This role focuses on assessing patient needs, developing and implementing care plans, and ensuring comfort and quality of life for patients and families at end of life.

The position is Monday through Friday, 8:00 AM to 4:30 PM, with compensation based on experience ($93,600–$97,760 annually).

Qualifications

• Minimum of one year in an acute care setting or comparable work experience required.
• Minimum of one year oncology and home care experience preferred.
• Minimum of one year Hospice experience preferred.

• Assess patients' needs, develop and implement care plans, and ensure the highest level of comfort and quality of life for patients in their end-of-life journey.
